Our goal is to ensure a smooth, compassionate transition into hospice care while supporting both the patient and their loved ones throughout the process.

Our admissions team carefully reviews the referral information and determines whether hospice services may be appropriate based on the patient’s condition and needs. If additional information is needed, a member of our admissions team may contact the referring provider or facility to confirm patient details and coordinate the evaluation process.

Step 2: Patient and family communication

Our team connects with the patient and their family to explain hospice services, answer questions, and ensure they understand the care and support available to them. A hospice nurse or clinician will perform an evaluation to determine whether the patient meets eligibility criteria for hospice services and to assess the patient’s needs.

Step 3: Hospice Evaluation / Admission

If hospice care is appropriate and the patient or family chooses to proceed, our team coordinates the admission process. Hospice services can often begin the same day or within 24 hours of referral.
