Our mission at Tranquil Oaks Hospice Care is to provide individualized and ethical care to meet the physical, emotional, spiritual, and practical needs of people with terminal illnesses. Tranquil Oaks reflects a spirit and philosophy of caring that emphasizes on comfort and dignity for all patients. We make it possible for the patient with life threatening illness to remain independent for as long as possible. We focus on control of pain and other symptoms, in familiar surroundings. Hospice care is centered on the patient, family, and caregiver. Hospice personnel respect and respond to the unique differences in family or caregiver lifestyle, values, and wishes.
Using an interdisciplinary team approach, Tranquil Oaks personnel (physicians, nurses, social workers, home health aides, volunteers, spiritual counselors, bereavement counselors, and others) provide palliative care in the patient’s home, mobilization, coordination of ancillary services, and bereavement support.
Tranquil Oaks Hospice Care affirms life. We believe that through appropriate care and the promotion of a caring attitude, patients, families, or caregivers are free to attain a degree of mental preparation for comfort that is satisfying to them.
